REDEMPTIONS

In order to redeem a State of Israel bond, certain forms are required. To determine which forms apply to you please see the below scenarios.

Note
If your bond was issued in certificate format and you have misplaced/lost the original physical bond you will need to complete the Affidavit of Loss & Indemnity in addition to the forms listed below. Certain fees will apply to replace a lost bond.

IMPORTANT: Once Computershare issues a payment, the check CANNOT be issued in a different name. Please make sure you submit the proper forms at time of submission.

Straight Redemption
(where the redemption check is issued exactly as the bond is now registered to the address on the account)
• Letter of Instructions (Simply, a letter indicating that you’d like to redeem your bond and where the check should be issued)
W-9 Form
• Bond Certificate(s)

Decedent Redemption/Transfer
(check/bond issued to beneficiary, non Joint Tenant Registration)
• Letter of Instructions (Simply, a letter indicating that you’d like to redeem/transfer your bond and where the check should be issued)
W-9 Form
• Bond Certificate(s)
Assignment Separate From Bond Form (must be medallion guaranteed)
Affidavit of Domicile (must be notarized)
• Original copy of Certified Death Certificate(s). If you submit an Assignment Form medallion guaranteed, a certified death certificate is not needed.

Joint Tenant/Decedent Redemption/Transfer (check/bond issued to surviving Joint Tenant)
• Letter of Instructions (Simply, a letter indicating that you’d like to redeem your bond and where the check should be issued)
W-9 Form
• Bond Certificate(s)
Affidavit of Domicile (must be notarized)
• Certified Death Certificate(s)

Redemptions for Custodial Accounts (Minors): Upon reaching the age of majority a bondholder needs to provide the following:
Assignment Separate From Bond Form (must be medallion guaranteed)
• Letter of Instruction (Simply, a letter indicating that you’d like to redeem your bond and where the check should be issued)
• Proof of age (drivers license, birth certificate or passport). Only required if an assignment form with medallion guarantee is not submitted. We will not accept a copy of a driver’s license or passport. The birth certificate must be original or a copy affixed with a medallion signature guarantee stamp.
W-9 Form (completed and signed by bondholder whose has now reach the age of majority)
• Bond Certificate(s)
NOTE: A copy of a driver's license is not sufficient.

Custodial Redemption (Bondholder is still a minor)
• Letter of Instruction
W-9 Form (complete with minors SSN# and signed by custodian)
• Bond Certificate(s)

Redemption for Minor (Where the bond is not registered in Custodial Form)
• Letter of Instruction
W-9 Form (completed with minors SSN# and signed by custodian)
• Proof of age (drivers license, birth certificate or passport)
Affidavit of Custodian (must be medallion guaranteed)
• Bond Certificate(s)

It is recommended that you send any bond certificates via registered or certified mail with a return receipt.

Once material is received and processed, a check will be mailed to the address of record. Please allow up to a maximum of 3 weeks to receive the check.

If your address has changed, please call Computershare @ 866-764-3425 or www.israelbondsdirect.com to update your current address.

State of Israel bonds may be transferred in limited circumstances. Please review the prospectus for your bond issue or call customer service (888-519-4111) to determine if your bond is eligible for transfer.
