1. Have you made a purchase within the past three years?

(which means you have already completed the required customer information form and can move onto question 2)

(which means you are either a new customer or haven’t purchased with us in some time. If this is the case you must answer the following questions)
Who is making the actual purchase?
an Individual
(an individual is someone who has a Social Security Number)
a Corporation
(an entity i.e., a trust, a company, a pension fund)
Are you a FINRA* affiliate (Someone related to or employed by a FINRA broker / dealer)?
*FINRA= Financial Industry Regulatory Authority (formerly NASD)

(the person who is purchasing the bond is either a member of FINRA or related to someone who is)

2. Who is the bond being purchased for?
an Individual
(an individual, over 18 years of age.)
a Corporation
(an entity other than an individual … in this case you will need to include documentation to support you have authority to purchase on behalf of the entity … i.e., trust agreement, corporate resolution, etc.)
a Minor
(an individual, under 18 years of age. Must have custodian information, only one custodian per minor.)
your IRA account
(purchases to an IRA require the funding to come directly from the IRA account.)
3. Are you funding part or all of a purchase with a matured bond?
